Experimental study of the Resistance of Transplanted jejunal mucosa in the Conine stomach


Abstract
Marginal ulceration continues to constitute a distressing complication following surgery for peptic ulcer. The intrinsic intestinal resistance to acid-peptic ulceration is considered one of the factors causing marginal ulcer.
The experiment was designed to evaluate the mucosal resistance to acid peptic digestion following the platransntation of the jejunal mucosa into the stomach antrum after removal of the antral mucosa. The dogs were, devided into two groups, In the first group, the jejunal mucosa was transplanted into the stomach antrum and allowed to come into contact with gastric contents initially. In the second group, around 10 cm. of mucosa was isolated from the muscle and serosal of the jejunum and kept the continuity of;the mucosa with the intact jejunum. The isolated mucosa was grafted into the stomach antrum and with the segment of jejunum Roux-en-Y gastro-jejuno-stomy was performed. The transplanted jejunal mucosa was, then prevented from contact with, the gastric contents using gastroentostomy in proximal stomach, and a continuous suture of 100 silk was placed between the graft site and gastroenterostomy.
The results were as following:
1. In the first group, the transplanted jejunal mucosa was observed at the end of l week `folio wing operation and no grafted jejunal mucosa were found.
2. The second group showed that jejunal mucosa was successfully grafted at the end of I week following operation. The continuous suture of 1-0 silk placed between the graft site and gastro enterostomy was removed in order to allow the gastric content to contact the transplanted muc-osa. At the end of 4 weeks following the second operation, no changes of transpalanted mucosa
were seen either grossly or microscopically regarding ulceration.
